Information Collection and Use

General: Alere Wellbeing collects personally identifiable information when you use certain features of our web sites. Alere Wellbeing uses the information collected to fulfill your requests for information, products and services, including fulfilling your online requests, facilitating customer service and informing you of our services. We do not rent, sell or otherwise provide personal information to third party marketers. In rare cases we may be required to disclose personally identifiable information for legal reasons; for example, to respond to a subpoena, summons, search warrant, court order, judicial process or legal requirement of a governmental agency. Disclosures may also be appropriate to protect our legal rights, maintain the security or integrity of our web sites and technology systems, or to avoid liability. 

Registered Program Participants: For individuals who register for our programs via our registration web sites or via other means, we also collect and use personally identifiable information as set forth in our Participant Privacy Notices . Alere Wellbeing takes steps to protect the privacy of information provided to us by our registered participants and that we create on their behalf. This information may include both health information and personal information. Alere Wellbeing adheres to requirements which govern security and privacy of personally identifiable health information outlined by the Health Insurance Portability and Accountability Act of 1996 (HIPAA – United States) legislation, the Personal Information Protection and Electronic Documents Act (PIPEDA - Canada), and the EU Safe Harbor Privacy Policy. Web Log File Data

We collect basic Web log file data about web site visitors. This information includes domain names, web site traffic patterns and server usage statistics. This information is used for site management and administration and to improve the features and content and overall performance of our site.

Data Security

Alere Wellbeing makes every reasonable commercial effort to ensure that all of the transactions that occur using our web site are secure. We encrypt your personally identifiable data with Secure Socket Layer (SSL) encryption to protect information transmitted and submitted on our site. (SSL is the industry standard protocol.)

Email Communication

Alere Wellbeing does not collect your e-mail address unless you voluntarily submit it to us or choose to communicate with us via e-mail. We do not sell or rent any e-mail addresses. When you send email to Alere Wellbeing via an Alere Wellbeing site, your message and e-mail address will be forwarded to the appropriate department or program within Alere Wellbeing.

Subscriptions

If you subscribe to a newsletter or other publication distributed by Alere Wellbeing via e-mail, we will keep your e-mail address in a private distribution list. We do not use this e-mail address list for any other purpose, and we only add individuals at their request. At any time, you may opt out by using the unsubscribe option provided on the email and on our web site, which will remove your information from the distribution list for that publication.

Third-Party Service Providers

Alere Wellbeing may engage third-party vendors to help us manage our web sites and assist us to better serve our registered participants and visitors. Alere Wellbeing does not share personally identifiable information with third party service providers, except to provide services to you or as permitted or required by law. One such vendor is ClickTale web analytics service. ClickTale may record mouse clicks, mouse movements, scrolling activity as well as text you type in this web site. This site does not use ClickTale to collect any personally identifiable information entered into this web site. ClickTale does not track your browsing habits across web sites which do not use ClickTale services. You can choose to disable the service at: www.clicktale.net/disable.html.

Deletion and Removal of Information

General: Non-identifiable information in our aggregate database will not be deleted. Like all responsible Web site publishers, Alere Wellbeing maintains backup files as a protection against natural disasters, equipment failures, or other disruptions. Backup files protect you and us because they lower the risk of losing valuable data. Removing a record from our main files does not remove that record from any backup systems. Backup records receive the same privacy and security protections as our other records.

Registered Program Participants: Identifiable Information regarding our registered participants is treated and retained in accordance with HIPAA.

Policy Changes

Changes to our privacy policy will be posted on all web sites owned by Alere Wellbeing.
